We are committed to providing our members with the healthcare and support services they require in order to promote our members’ involvement in decisions about their health needs. That is why we offer different healthcare benefit options that put you in control of your health care.

We are pleased that you have chosen Commonwealth Care Alliance for your care needs. Our Member Services Staff are available to speak with you Monday through Friday, 8:00 am – 5:00 pm. The Senior Care Options (SCO) program was designed to help elders remain as independent as possible. The SCO program provides a Primary Care Physician (PCP) or Primary Care Team (PCT) to coordinate a comprehensive range of health and health-related services that best suits a person’s health care needs. SCO PCPs are dedicated to providing a personalized approach to health care so that members, their families, and the CCA network of providers are able to work together efficiently and satisfactorily on the member’s behalf.

The Commonwealth Care Connection (CCC) program was designed to help those with disabilities receive proactive health care planning and early intervention to minimize costly hospitalizations and nursing facility admissions. CCC PCPs are dedicated to providing a personalized approach to health care so that members, their caregivers and the CCA network of providers are able to work together efficiently and satisfactorily on the member’s behalf. The program strives for increased consumer and caregiver satisfaction through the promotion and integration of consumer involvement in all aspects of care.
